Access Medical Records & Images
UW Medicine MyChart is a free, secure and convenient way to access many types of personal health information in your inpatient and outpatient medical records, including test results, medical history, medications, immunizations and more.
If you need more information than you can find in MyChart, you can easily request copies of your UW Medicine medical records and radiology images.
Patient confidentiality
When requesting records on behalf of a patient, please include a copy of your Power of Attorney. To read more about how UW Medicine protects and discloses your information, please review the Joint Notice of Privacy Practices.
Medical records
Request your medical records
Option 1: Request a copy of your medical records via MyChart. When requesting through MyChart, a copy of your medical records will be released to MyChart and will be available for viewing/downloading for 30 days from the received date. Please allow 15 business days to process your request.
Option 2: Complete and sign a Patient Authorization to Disclose, Release and/or Obtain Protected Health Information.
Mail, fax or email your authorization form or request to the location where you received care (see below). Please allow 15 business days to process your request.

Pay for your medical records online
You can now pay for your medical records online through our UW Medicine Medical Records Invoice Payment Portal via credit card (Visa, Mastercard, Discover, and American Express).
Reference the medical records invoice to pay online and include the following:
- Requestor Email Address*
- Requestor Name/Company Name*
- Release ID*
- Invoice Number*
- Patient Last Name*
- Person Authorized This Payment*
- Comment (Optional)

Request a correction or amendment to your medical records
If you believe there is an error in your medical record, you may request an amendment by completing the Request for Correction or Amendment of the Medical Record.

Access your medical records online
Sign up for MyChart to see your outpatient and inpatient records online. Login to the MyChart patient portal to access your medical records from your clinic visits or hospital stays at Harborview Medical Center, UW Medical Center – Montlake, UW Medical Center – Northwest, UW Medicine Primary Care and other UW Medicine affiliated clinics. Not all of your medical records are available through MyChart.
To request additional medical records, contact the location where you received care (see below). Please allow 15 business days to process your request.

Requesting records for another person
If you are not the patient, we will need you to provide more information about your right to receive the records of someone else.
We may ask for items like a Power of Attorney or, if the patient is deceased, a death certificate.

Charges for medical records
Continuity of care (medical records sent to a physician, clinic, hospital or other healthcare provider: No charge.

Obtain a copy of a birth or death certificate
Contact the King County Vital Statistics for birth and death certificates at 206.897.5100 or visit the Washington State Department of Health for more information.
Request an EHI Export
UW Medicine uses an electronic health record called Epic that is certified by the Office of the National Coordinator (ONC) Health IT Certification Program. The ONC Certification Program requires that electronic health information (EHI) stored in Epic to be made available to patients in a machine-readable format commonly referred to as “EHI Export.” To learn more about what an EHI Export would contain, or to request this, please use this form: Patient Authorization to Release a Machine-Readable Export of Protected Health Information.
